You need to add to this some other proof of pregnancy with a doctors signature like an ultrasound or your pregnancy followup or tipat chalav card. Only a licensed midwife can supply you with all these documents. You do not need to go to the hospital in order to register. However, if you want to get the maanak lida then you need to go to hospital within 24 hours and be admitted for 12 hours.
